I____________________________________ hereby authorize HardworkHardbodies to act on my best judgment to in any emergency requiring medical attention and waive and release any and all rights against HardworkHardbodies and the host school/business site or any other sponsors associated with this camp, clinic, or private training session. I grant full permission to the host school/business site and/or sponsors to videotape, photograph, or record this event for legitimate training or marketing purposes. There are no make-up sessions for classes or large groups. Please be advised  that  monthly payment sessions are non-refundable. Accounts are non-transferable.
